Once you have an appointment, you may need to obtain authorization from your primary care physician (depending on your insurance plan). Our scheduling staff will know what authorization, if any, need to be obtained. Please understand that it is the patient's responsibility to obtain authorizations.
Please bring your insurance card to your appointment. We will file all insurance claims.
Please come to your appointment prepared to pay your co-pay, your deductibles and other patient responsibility amounts. These payments are due at the time services are rendered. We accept cash, checks, Visa, Master Card and Discover.
